Ursa and Ozai, I don't imagine it was arrange marriage. Actually in my opinion the two families probably didn't get along at all but the Royal Family didn't want to get out the descentants of the Avatar so they were tolerated.
Ursa had met Ozai when they were just small children and thought he was arogant however being geniune kind person and a bit naive she wanted to patch things between the two families and find that there was a kind spirit in Ozai and fell in love and married him. Despite the pleas of her parents, I imagine it wasn't until her children were older that Ozai showed his true side.
I can imagine they were happy once, that she was happy.

And Zuko also mentioned in a chapter that his father hadnt been there /some kind of Sumer resort/ since their family isnt happy... or something like that. - I still should watch the series in english...
